What is Hallucination?

A hallucination is when an AI model produces confident but false or fabricated information.

Because models generate plausible text, they can state wrong facts with total confidence. Hallucination is the risk of trusting fluent output that isn't grounded in reality.

Grounding techniques like RAG, tool-calling for real data, model routing to stronger reasoners, and human review all reduce hallucination — which is why agents that act should verify against real sources, not just assert.
